Below are helpful instructions and tips on how to use the Bearcats Health App and fill out the required University of Cincinnati Standardized Immunization Form.
Your compliance status from Med+Proctor will be transferred to the Bearcats Health App as of 9/18/23, when Med+Proctor becomes unavailable.
If you are non-compliant as of 9/18/23, you will be required to submit a fully completed immunization form to the Bearcats Health App to become compliant. You can only use the University of Cincinnati Standardized Immunization form. No other forms will be accepted.
Unfortunately, your previously uploaded documents will not transfer from Med+Proctor to the Bearcats Health App, only your compliance status. We apologize for the inconvenience.
If you had a previous exemption in Med+Proctor for any vaccine, you will need to re-submit through the Bearcats Health App exemption process.
If you had immunization documentation in Med+Proctor that you want to access, you will have to pay the $10 fee for Med+Proctor Pro in order to download your records and use them whenever you need them. However, your documents from Med+Proctor will not be accepted in the Bearcats Health App, as the Standardized Immunization Form must be used, so you will still have take your documents to a medical provider to have them review, fill out your form, and sign it.
Tips on filling out the Standardized Immunization Form so that the Bearcats Health App system can properly read it:
Signature validation requirements for all Standardized Immunization Forms include one of two options:
PDF and image files are able to be used. Those are the only two file types that will be accepted. You can either scan your form or take a photo of your form with your phone. If you have image files, the Bearcats Health App system will prompt you to merge them and it will convert them to a PDF file for you.
If you are in process with a vaccine series, you are able to submit partial forms with the information for each new dose as you move through the series, but you must include all previous doses of the series each time you submit a form.
For example, if you are in process with a Hepatitis B vaccine, which is a 3-dose series, you should include your first dose on your initial form. You will need to wait four weeks to have your second dose. At that time, you may upload a form that just includes dates for your Hepatitis B vaccine, but you must include the date of your first dose as well as the date of your second dose in order for the Bearcats Health App system to read your form. You will then have to wait five months from your second dose to receive your third. At that time, you may upload a form that only includes dates for this Hepatitis B series, but you must include the date of your first and second doses along with your third/final dose.
Health Professions Student Specific Requirements
All Health Science students are required to complete an annual TB screening questionnaire and receive a flu vaccine no later than 11/01/2023. You will be marked as non-compliant until these are completed in the Bearcats Health App.
